Reserve Request Forms

Below please find forms instructors will use to place materials on Reserve. Please save copies of your forms and letters until the end of the semester for your records. For more information, questions, or comments, contact Jilda Elk or Cheryle Holeman at the Library Circulation Desk or Reserve Office.

The forms are offered in PDF and Word formats. For best results, Netscape users hold down the Shift key while clicking the link to download files. Internet Explorer users should right-click on the link, then choose Save Target As... to download files. See [ TIPS] for hints on using these two file types.
