PETITION FOR POLICY EXCEPTION
THIS FORM IS TO BE USED IN THE CASE OF PROCEDURES WHICH MAY VARY FROM CATALOG OR HANDBOOK SPECIFICATIONS OR ESTABLISHED UNIVERSITY POLICIES.
- Complete top part of form.
- Include all relevant information. (For example: course number and title, term, grade, and instructor.)
- Obtain appropriate signatures. Signature of Major Department Chairman/Dean is always required except for petitions requesting section changes for courses not in your major department.
- Return all copies to the Registrar's Office.
- Please allow a minimum of four weeks for processing.
